Evidenced-Based Practices Portfolio Project (TOTAL 300 points). Write a one page summary describing what makes a practice evidence-based. Review evidence-based practices for transition. Identify three practices for early childhood transition and three practices for secondary transition. In approximately one page per practice, thoroughly describe each of the six practices including how it would be implemented and who might be involved in the implementation.
